  • Latest Report Card (F138) and Form 137
  • Certificate of Good Moral Character (GMC)
  • Satisfactory DGS Entrance Examination and Personal Interview
  • Duly accomplished ACR Form (Foreign students only)
  • Study permit (For foreign students only)
